How to Register 

You can register as a patient at Cloughmore by either printing off the registration forms below or by coming into the surgery to pick up the forms in person. To find us please click on the button below. 

Information for Registering Patients

The new registration forms are available from reception. Once completed return to the reception desk. The receptionist can check you have completed this correctly and answered all the questions. It can take up to one week to register a new patient.

If you move out of the practice area you will be removed from the list and you will have to register with a more local practice.

A practice information leaflet is available from reception.

Adult Registration (18+)

Once the forms have been filled in please bring them to the surgery along with ID* and a receptionist will look through the forms to see if everything has been filled in correctly, do not post them into the surgery as if they have not been filled in correctly it will delay your registration.

*ID - this can any form of identification e.g., Drivers Licence or Letter with name and registering address.

This is the main NHS registration form. Please make sure you fill out all the information you are able to. Reception will check this form when you hand in.

It is very important we collect up-to-date health information on registration. Please fill out all information that pertains to you as honestly as possible. 

We ask all adult patients to read and sign this patient declaration. This details our ZERO-TOLERANCE and  BENZODIAZIPINE policies. 

Children Registration (17 and below)

If you are bringing in a registration for a child please bring in their Personal Child Health Record (red book) so we can take a copy of their immunisations this is to make sure they do not miss any scheduled immunisations while we wait for their records to come over to the surgery.

We are only able to accept registration forms for children if there is an adult registering with the child or if there is a responsible adult already living at the registering property.

To allow us to able to make sure that your child is up-to-date with their immunisations please fill out the form attached.
